Oral Surgery

歯科ハミール

Hearing “oral surgery,” many people probably picture “having a wisdom tooth pulled” or “surgery that cuts into the gum,” and fear and anxiety keep them away from treatment.

However, 歯科ハミール in Handa holds to the guiding principles of “caring about pain, saving teeth wherever possible, and not extracting lightly,” and we almost never push treatment forward before the patient has agreed to it.

Even if a wisdom tooth hurts, we don’t jump to the easy conclusion of “let’s just pull it now.” We first run a detailed examination with digital equipment, assess the overall balance of your mouth, and carefully judge, from a professional standpoint, whether “extraction is really needed now” or whether “keeping it would be better for the future.”

Please don’t hold back thoughts like “I’m afraid of surgery” or “I’d rather not have it extracted if possible” — tell us. We take your feelings to heart and think through a lower-burden treatment plan together with you.

That Anxious Feeling of “I’m Scared to Have It Extracted…”

Criteria for Whether a Wisdom Tooth Should Be Pulled or Kept

[When to Keep It] It Has Grown In Straight and You Can Brush It Thoroughly

If a wisdom tooth has erupted normally and straight, just like your other teeth, and daily brushing keeps it free of plaque (with no concern about cavities or periodontal disease), there’s no need to force an extraction.

When there is no pain or swelling and the tooth is being kept clean

[When to Keep It] It Could Serve as a “Foundation” for Another Tooth in the Future

If a molar in front of it is lost later on, a healthy wisdom tooth that has been kept can potentially serve as an anchor for a restoration called a “bridge.” To keep more options open for future treatment, we sometimes deliberately preserve the wisdom tooth.

When it can be used as an abutment tooth in the future

[When to Extract It] Gum Swelling and Pain Keep Recurring Over a Short Period

If a symptom like “the wisdom tooth only swells when I’m tired” keeps recurring within a short cycle of 1–3 months, it means bacteria are multiplying inside. Left untreated, it can worsen to the point of facial swelling, so extraction is chosen.

When the gum repeatedly swells and subsides over a short period

[When to Extract It] Debris Easily Builds Up Against the Tooth in Front (Cavity Risk)

If a wisdom tooth has erupted at an angle, food debris tends to get wedged between it and the healthy tooth in front of it. To keep that from dragging the important molar in front into a cavity or periodontal disease as well, the wisdom tooth causing the problem is extracted.

When food tends to get caught between the wisdom tooth and the tooth in front of it

[When to Extract It] It Pushes and Disrupts the Whole Bite

If a wisdom tooth erupts while pushing forward with strong force, it can knock the front teeth out of alignment like dominoes, one after another. To protect a beautiful, well-aligned bite, extraction is chosen.

When there is no space secured for the wisdom tooth to come in and we judge that it would disturb the normal alignment of your teeth

[When to Extract It] There Is No Opposing Tooth, and It Bites Into the Gum or Cheek

For example, when only the upper wisdom tooth has erupted with no corresponding tooth below, the upper tooth keeps growing longer and injures the lower gum or the inside of the cheek, causing pain. Since it also serves no function in chewing, it is a candidate for extraction.

When the upper and lower teeth do not bite together

The Real Danger of Leaving a Swollen Wisdom Tooth Untreated

“The pain will go away after a while, so it’s fine” — enduring a swollen wisdom tooth and leaving it untreated is extremely dangerous. If bacterial inflammation spreads from the jawbone to the area around the neck, it can trigger a serious infection called “cellulitis.” In the worst case, pressure on the airway can cause breathing difficulty and even become life-threatening.

The area around a wisdom tooth tends to swell up suddenly when your immunity drops due to work fatigue, lack of sleep, a cold, and so on. The moment you feel “something’s a little off,” it’s important to see us early, before the pain gets worse.

[A CT Scan for Safe Extraction]
To keep the risk of extraction to a minimum, 歯科ハミール in Handa is equipped with “dental CT,” which lets us grasp the condition of bone and nerves in three dimensions. We can check, to the millimeter and in advance, the distance between an upper wisdom tooth and the sinus (the maxillary sinus), and the positional relationship between a lower wisdom tooth and the large nerve nearby (the inferior alveolar nerve). This prevents serious complications such as “fluid leaking from the nose after extraction” or “lingering numbness in the lip” before they happen, making for extremely safe surgery.
